Not done so myself, but I gather MS Publisher can do the job, if you have access to Office that has Publisher included.
A quick internet search for Publisher A5 Booklet seems to throw up a fair few walkthroughs also.

I'd go with something like a 135gsm silk index board self cover, or if you want a separate cover, maybe use a 190gsm for the cover and 115gsm for the internal pages. All descends on what type of work you're producing and its intended use/audience to be honest
