Other than them, Manchester and the Madrids, I'm struggling to think of another city that has two massive sides.

The thing is, it's only recently that countries have had more than one entrant. Until the birth of the so-called "Champions" League, each country would have one entrant. So, that explains why it has only happened twice.
And what about Cardiff City and Inter Cable-Tel?
