I am quite shocked by viewing this. Didnt see it at the time but it is obviously premeditated violent conduct - he stands waiting for Cahill to run by before elbowing him. If Cahill had made more of it at the time the ref may have taken action but possibly he didn't see it ?. If not, I dont understand why it hasnt been reviewed.
