Whilst clearly the ultimate responsibility for the terrible events lies with the murderer it was enlightening to listen to the chairs damning report
Basically the police , ambulance and fire service big wigs appear to all have been culpable in a catalogue of schoolboy errors and a failure to learn from training and previous incidents
It appears these agencies failed almost completely and there was no coordination between them
Inadequate , ineffective , failure to help those we serve etc
Apologies all round but there should be resignations and sackings if there havnt been already
